Services | Patient Discount Program

At Ohio Hills Health Centers, we believe that cost should never be a barrier to receiving quality healthcare. The OHHC Patient Discount Program is designed to help patients who may not qualify for Medicaid but still struggle to afford the care they need. Using a sliding fee scale based on household size and income, the program can significantly reduce the cost of medical and dental services for eligible patients. All information shared as part of the application process is kept strictly confidential.


Who Is Eligible

The Patient Discount Program is available to patients whose household income falls within federal poverty guidelines, which are updated each January. Eligibility is based on your household size and gross income, and the program uses a sliding scale — meaning the discount you receive is proportional to your financial need. Patients who have Medicaid or Managed Medicaid coverage are not eligible for the program, as those plans already provide discounted or no-cost care.

New patients may receive a one-time discount on their first visit by completing a self-declaration at the front desk, even before a full application has been processed.


What Is Covered

The Patient Discount Program applies to medical services and most dental services provided at OHHC facilities. Please note that certain dental procedures are excluded, including lab fees associated with partials or dentures. If you have questions about whether a specific service is covered under the program, our patient accounts team is happy to help.


Prescription Assistance

In addition to discounts on clinical services, the Patient Discount Program includes prescription assistance through a network of contracted pharmacy partners. This ensures that patients can access the medications they need at a reduced cost as part of their overall care. Ask our team for a current list of participating pharmacies in your area.


How to Apply

Applications are available at any OHHC location — Barnesville, Freeport, Quaker City, and Woodsfield — or can be downloaded from our website. To complete your application you will need to provide:

  • Your household size and contact information
  • Gross household income details
  • Acceptable income verification documents
  • Your signature

Once submitted, your application will be reviewed and you will be notified of your discount level. Our patient accounts team is available to walk you through the process and answer any questions you may have.
