Healthcare is about more than what happens inside the exam room. Ohio Hills Health Centers offers a broad range of education and support services to help patients and community members navigate their health, connect with local resources, and make informed decisions about their care. Whether you need help understanding a diagnosis, locating community programs, or coordinating a specialist referral, our team is here to support you every step of the way.
Health Screenings
Preventive screenings are one of the most effective tools for catching health conditions early, when they are most treatable. OHHC offers a community health screening schedule throughout the year, giving patients and community members access to important screenings without needing a full clinical appointment. Call (740) 239-6447 to learn about upcoming screening events and to schedule your spot.
Community Resource Guide
Finding the right support can be overwhelming, especially when you are unsure what is available in your area. Our Community Resource Guide is a comprehensive listing of programs and services available across the five counties served by Ohio Hills Health Centers. From food assistance to transportation to mental health resources, the guide helps connect patients and families to the local support they need.
Health Education
Staying informed is an important part of staying healthy. OHHC provides health education resources, including video content covering a variety of wellness topics, to help patients better understand their health and make confident, informed decisions. These resources are available to all patients and community members.
Advance Directives
An advance directive allows you to document your healthcare wishes in advance, ensuring that your preferences are known and respected if you are ever unable to speak for yourself. OHHC provides access to advance directive resources for any individual age 18 or older, including:
- Advance Directive Packet — covers Living Wills, Health Care Power of Attorney, Anatomical Gifts, and Ohio DNR laws
- State of Ohio Advance Directive Forms — official state forms for documenting your healthcare decisions
Our team can help you understand these documents and ensure your wishes are properly recorded. Ask at any OHHC location for more information.
340B Drug Pricing Program
As a Federally Qualified Health Center, OHHC participates in the federal 340B Drug Pricing Program, which makes certain medications available at significantly reduced costs. This benefit is available to all OHHC patients regardless of insurance status or ability to pay, helping to ensure that cost is never a reason to go without needed medication. Ask your provider or our pharmacy team for more details.
Referral Coordination & Prior Authorization
When a specialist referral or prior authorization is needed, our staff handles the process on your behalf so you can focus on your health rather than paperwork. Our team coordinates referrals with timely transfer of documentation and appointment verification, and works directly with insurance companies to obtain pre-approvals for medications, specialist visits, and procedures. Our goal is to minimize delays and keep your care moving forward without unnecessary interruption.
Interpreter Services
Effective communication is essential to good care. OHHC offers sign language interpretation and language translation services to ensure that every patient can clearly understand their care and communicate with their providers — regardless of hearing ability or primary language. Please let our team know in advance if you require interpreter services so we can make the appropriate arrangements for your visit.
For more information about any of our education and support services, call (740) 239-6447 — answered 24 hours a day — or ask a member of our team at your next visit.
